• Be 18 years old and be authorized to work in U.S.
• Must have current Driver's License / Photo ID
• Social Security Card
• Must pass Level 2 Background
• Must be Free of Communicable Diseases
• AIDS/HIV Training (1 hour)
• Dementia/Alzheimer's Training (3 hours)
• CPR First Aid
• Pass Drug Screening 
• Current Auto and Vehicle Registration (if providing transportation)
• References (3)
